This visa is also called the “Limited Stay Visa.”
To initiate and to expedite the process of the Authorization, it is advisable that the sponsor/individual/organization in Indonesia submit the request for the applicant’s authorization from the Directorate General of Immigration in Jakarta, Indonesia.

If you’re inside the U.S, you can find your “jurisdiction” based on the State you live in here.
In many cases, the website that you find will be in the local Indonesian language. You will need to click a button (normally on the top right of the page) to translate the website to English.
Find a link somewhere on the page that says something like “Consular and Visas” or “Visas” or “Consulate Section.”
Find the link to “Limited Stay” to determine the required documents the Consulate requires to process your application.
The site for the Embassy of Indonesia in Washington, D.C. has a great list of the documents required for this visa.
Submit the required information (and fees) that you gathered in Step 5 to the consulate.
Congratulations! After a period of waiting, the Indonesian Consulate/Embassy will give to you your brand new Indonesia visa.
