As stated here, the D-6 or Press Visa for Cuba must be requested by you from the press office of the Cuban Embassy or Consulate. After getting approval, you will then need to visit the embassy/consulate during normal business hours.

If you’re inside the U.S.:

If you’re outside of the U.S.: Click here to go to the proper pages of the Cuban Mission website.

Submit your required materials and visa fee from Step 2 to the embassy or consulate you found in Step 1.
If you want to apply by mail, don’t forget to enclose a self-addressed prepaid return envelope.
Congratulations! The Embassy of Cuba will give to you your brand new Cuba visa.
- If you applied in person, go pick it up.
- If you applied by mail, wait until it gets to you.
